When someone fails to fill a 9x12 card or has a terribly difficult time doing it, you can almost count on one recurring pattern.... they're using one single channel to prospect and sell within. They're doing walkins only, emails only, phone calls only, networking only, etc. The problem is that this is a terribly ineffective and grueling way to fill a card.
